Research Abstract	ラットにdexamethasone(Dex)を慢性的(4週間)に投与した場合,体重減少,骨の成長障害,骨塩量の低下,高コレステロール血症,免疫能の低下が引き起こされるが,これらのDexの作用は投与するDexの用量に依存していた。Dexに成長ホルモン(GH)を併用投与した場合,体重減少,成長障害,高コレステロール血症については顕著な変化はみられなかったが,骨塩量の増加傾向が認められた。また,Dexの慢性投与によって血中IGF-Iが著明に抑制されたが,GHの併用投与により,IGF-Iの上昇が認められた。免疫系については,Dexの慢性投与によって脾リンパ球数が減少し,血中の免疫グロブリンも低下した。興味あることには,GHの併用投与を行った処置群では,脾リンパ球のサブセットにおいてCD4陽性細胞数とCD8陽性細胞数の比(CD4/CD8)が有意に上昇していた。しかし,GHの単独投与群ではCD4/CD8は対照群と差がなかった。一方,血中の免疫グロブリンに対してはGHの併用投与の効果は認められなかった。以上の結果から,GHの併用投与はステロイドの慢性投与の結果生ずる骨塩量減少に対してある程度の有効性があると考えられた。一方,GHはステロイド投与下においてリンパ球サブセットのCD4/CD8を上昇させる作用があることが今回明らかとなった。このことはGHの併用投与がステロイドの免疫抑制作用に拮抗し,臨床的にGHを使用する際には原疾患の活動性を亢進させる可能性を示唆している。従って,今後のGHの臨床応用については,GHによる免疫系に対する抗ステロイド効果を考慮して,適応症例を慎重に選択する必要があると考えられる。さらに,GHの免疫系における抗ステロイド作用に関する分子機序の解明が今後の重要な基礎的検討課題であると思われる。
