Research Abstract |
The effects of retinoic acid (RA) on ductal contractility were investigated in rat fetuses. On the 21st day of gestation (near-term), the inner diameters of the ductus arteriosus(DA) and main pulmonary artery(PA) were measured to get the inner diameter ratio (DA/PA) of the fetus, following maternal ingestion of 1mg/Kg of indomethacin (Ind) 4hours before study. DA/PA is 1.0 in the fetus without medication. DA/PA decreased to 0.54 with Ind in the normal fetus. Stronger constriction occurred with Ind after two days' administration of RA (10mg/Kg) on days 19 and 20 (late fetal stage), with DA/PA decreased to 0.31. Retinoic acid, administered to pregnant rats in late gestation, increases ductal contraction by indomethacin. Vitamin A has the same effect, and its clinical dose(1mg/Kg), administered 1 and 2days before term increases fetal ductal constriction by indomethacin in near-term fetal rats. Retinoic acid and vitamin A themselves do not constrict the fetal ductus artenosus. It remains to be checked clinically if aspirin, ibuprofen and indomethacin cause more fetal ductal constriction following intake of increased vitamin A in pregnant women. Clinically, indomethacin treatment of the patent ductus arteriosus in premature infants may not be effective in some cases. It remains to be studied clinically if pretreatment with vitamin A increases ductal constriction in the premature baby with indomethacin.
