| Research Abstract |
家族性大腸腫瘍症(FAP)に関して、既に明らかにした70家系の生殖細胞突然変異に加え、今回新たに7家系の生殖細胞変異を同定した。
遺伝性非腺腫性大腸癌(HNPCC)に関して、今回新たにミスマッチ修復遺伝子hMSH6遺伝子の生殖細胞突然変異(codon534 C deletion)を持つ日本のHNPCC家系を見い出した。この家系では、hMSH2やhMLH1遺伝子変異を持つ家系に比べてやや高年齢で卵巣癌、子宮癌、大腸癌が発生する傾向が見られた。家系内の1患者には、子宮体癌、表面型横行結腸癌とともに、表面型過形成性ポリ-プが約10個見い出された。子宮体癌と大腸癌ではhMSH6遺伝子の体細胞突然変異も検出され、遺伝子不安定性(RER)が認められた。大腸癌にはAPC、p53遺伝子変異は検出されず、Kras遺伝子変異、TGFβRII遺伝子(A)_<10>、BAX遺伝子(G)_8の突異変異が検出された。RERとTGFβRII遺伝子の異常は、調べた2個の表面型過形成性ポリ-プにも認められた。
生殖細胞突然変異が明らかになったFAPとHNPCC15家系の38人の家系員について、インフォームドコンセントに基づき末梢血DNAのPCR-SSCP分析により発症前診断を実施し、22人の保因者を見い出した。
