前立腺肥大症の病態を特定する症状及び排尿生理異常の解析

Research Abstract

昨年度作成した独自の前立腺肥大症スコア(SPSS)を前立腺肥大症患者30例に使用しIPSSとの相違を検討した。即ち年齢、エコー診断による前立腺容積、uroflowmetry,pressure/flow study,A-GノモグラムSchaeferノモグラムの閉塞パラメータや残尿量との相関を検討した。
uroflowmetryのQmaxとQaveはSPSSと有意な相関を示したが、IPSSとの相関はなっかた。SPSSの方が尿流動態即ち他覚所見を反映することが確認された。
SPSSもIPSSもA-Gノモグラム及びSchaeferノモグラムの閉塞パラメータと相関がなっかた。またtotal prostate volume及びadenoma volumeとも相関がなっかた。しかし、A-Gノモグラムはtotal prostate volumeとSchaeferノモグラムはtotal prostate volumeと相関を示した。
前立腺が肥大して下部尿路を閉塞することはA-Gノモグラム及びSchaeferノモグラムの閉塞パラメータと相関することでわかりやすいが、SPSSがuroflowmetryとのみ相関があったにもかかわらず閉塞パラメータとの相関がなかったことから今後も検討が必要である。
