| Research Abstract |
1)症例、標本の選別(清水)
手術術式、化学療法のプロトコールが一定した1980年から1995年の間に癌研究会附属病院婦人科で悪性卵巣腫瘍と診断された570症例を清水、秋山で再検討した結果、非上皮性浸潤癌、およびLow-malignant potentialを除く、上皮性卵巣悪性腫瘍(浸潤癌)は461症例であった。これらを今回の研究対象とした。各症例のH-E標本の代表的なslideを3枚以上選別した(清水)。
2)461症例の臨床データをコンピュータに登録(清水)
3)光学顕微鏡による病理組織学的検討(清水、秋山)
光学顕微鏡により、組織型を決定した。次いで、構造異型、核異型、及び核分裂数を計算し、以下の方法により、点数化した。(清水)。
a)構造異型:管状:1点、乳頭状:2点、充実性:3点
b)核異型:軽度:1点、中等度:2点、強度:3点
c)核分裂数(MF/10HPF):0-9:1点,10-24:2点,25以上:3点
Pearson's product moment correlation coefficientにより、相関係数を算出した結果、上記3因子は互いに独立した因子であることが明らかにされた。
4)上記3因子に基づく、新しい分化度診断法の作成
上記点数を合計し、総合点により以下の如く、gradingした。
Grade1(well differentiated):3-5点,
Grade2(modeartely differentiated):6-7点,
Grade3(poorly differentiated):8-9点
同じ治療方法および厳密なfollow-upを受けた461症例で、上記gradeと予後との関係を調査した。I,II期、III,IV期の2群に分けて、検討した結果、New Grading Systemは、早期、進行期とも、予後と有意の相関を示した。
5)卵巣癌の予後因子としての統計学的検討(清水)
461症例を進行期により層別化(I,II期、及びIII,IV期の2群)した上で、Kapla-Meier法により生存曲線を得、各予後因子の有用性をUnivariate分析(Logrank法)、Multaivariate分析(Cox' proportional hazards model)により検討する。予後因子として、I,II期では、全身状態、及びnew grade,III,IV期では全身状態、組織型、残存腫瘍径、化療の効果、及びnew gradeが統計学的に有用であった。即ち、本研究で考案されたnew grading systemは、早期、進行期ともに機能した。
