Research Abstract	膀胱がんに関する多くの臨床病理学的研究から、膀胱内にしばしば広汎な進展を示す上皮内がんは浸潤がんの前駆病変と考えられている。膀胱がんの上皮内進展の分子機構を解明することを本研究の目的とした。ヒト膀胱がん細胞株をSCIDマウス腎盂ならびに膀胱に移植しがん細胞の上皮内進展(IES:intraepithelial spreading)の有無を組織学的に検討した。本移植モデルにより、ヒト膀胱がん細胞株を腎盂・膀胱においてIESを示す群(IES細胞)と示さない群(非IES細胞)に分類できることがわかった。IES細胞・非IES細胞ならびに不死化移行上皮細胞の細胞形態・接着能ならびに運動能を細胞外基質上観察した。ラミニン上における培養で、IES細胞だけがコロニーをつくらず強く分散(scattering)する特徴を有していた。さらに、接着能・遊走能・運動能を詳細に検討すると、IES細胞だけがラミニンに対する高い接着能・遊走能・運動能を有していた。ラミニン受容体として機能するインテグリンの発現をウェスタンブロット法・免疫沈降法で検討したところ、IES細胞ではインテグリンβ_4の発現が著明に低下しており、インテグリンα_6β_1の発現が優位であった。膀胱がんの上皮内進展には、インテグリンβ_4の発現低下とそれに基づくラミニン上での運動能の亢進が関与していると考えられた。今後、インテグリンβ_4をIES細胞に遺伝子導入し、ラミニン上での運動能や移植モデルにおけるIESが抑制されるかどうか検討するとともに、ヒト膀胱がん手術材料の免疫組織化学的検討を行い、膀胱上皮内がんにおけるインテグリンβ_4の発現低下の意義について更に考察することを予定している。
