Research Abstract

モンゴロイドにはある種の難治性炎症性眼疾患が多発することが知られている。我々は過去一貫して、これらの難治性炎症性眼疾患の疫学、人類遺伝学、およびその分子遺伝学的発症機構の研究を行ってきた。
本年度はとくに原田病、ベーチェット病の臨床像を比較検討するとともに、分子遺伝学的発症機構を検索した。その結果、原田病の人類学的分布は東南アジア、中央アジア、更には北アメリカ、南アメリカにもみられた。しかし、白人には非常に稀であった。これに対しオーストラリア原住民には原田病の発症が確認され、ニューギニア原住民でも原田病の存在の可能性が示唆された。東アジアの原田病患者ではHLA-DR^*04との強い相関が見られ、同一のHLA相関のが他の人種でも確認されるかどうか、今後HLA-DNAタイピングを継続してゆく予定である。
これに対し、ベーチェット病は東南アジアにはみられず、シルクロード沿いの北東アジア、中央アジア、そして東アジア、地中海沿岸諸国に多発していた。とくに日本以外の多発国、例えば韓国、中国、イラン、トルコ、モロッコなどではベーチェット病の発症率がいまだに増加しており、その成因を詳細に検討中である。ベーチェット病の分子遺伝学的発症機構の解析については、本年度はギリシャ人における解析が終了した。その結果、ギリシャ人のベーチェット病はHLA-B^*5101頻度が81.1%と日本人よりさらに高頻度であった。MICA-A6頻度も正常ギリシャ人より著明に高く、我々が日本人で見い出した疾患感受性遺伝子相関が、ギリシャ人でも全く同一であった。本邦から遠く離れたギリシャにおいて、なぜ同一の遺伝子相関を示す炎症性眼疾患が見られるのか、今後の重要な検討課題である。
本年度は更にイランからもDNAサンプルを入手できたので、現在鋭意分析、検討中である。
