Research Abstract	家族のEE(expressed emotion)と気分障害(mood disorders)の経過についての知見は一貫しておらず,また,これについてのアジアからの報告はなかったことから,これらの関連を検討するためのコホート研究を行った.アメリカ精神医学会の診断基準(DSM-IV)と国際疾病分類(ICD-10)に従って気分障害と診断された32名の患者と36名のその主要な家族が研究対象となった.患者の入院後2週間以内に,その家族に対してカンバウェル家族面接(Camberwell Family Interview,CFI)による面接を行い,EE評価を行った.患者の退院後から9カ月間追跡するコホート研究を行った.CC(critical comments)とEOI(emotional overinvolvement)のいくつかのカットオフポイントで患者を高EE群と低EE群に分け,9カ月再発リスクを比較検討し,再発リスク比とその95%信頼区間を計算した.また感度(sensitivety),特異度(specificity),陽性反応的中率(positive predictive value),陰性反応的中率(negative predictive value)を算出し,最良のカットオフポイントを検討した.可能性ある交絡要因の影響をコントロールするために多重ロジスティック回帰分析を行った.その結果,CC3個以上あるいはEOI3点以上を高EE群,それ以外を低EE群としての9カ月再発リスクはそれぞれ83.3%(5/6),19.2%(5/26)であり,再発リスク比(95%信頼区間)は4.3(1.8-12.2)であった.この分け方での,妥当性の指標の値が最良であった.多重ロジスティック分析の結果でも,EEの影響は有意であった.日本においてもCFIによるEEは気分障害の再発に影響を与えることが示唆された.
