陽性荷電蛋白の腎尿細管基底膜への結合による過敏性間質性腎炎モデルの作製と解析

Research Abstract	平成10年度は、液性免疫移入により尿細管基底膜(TBM)上にin situ型免疫複合体形成を作製して過敏性尿細管間質性腎炎(TIN)を誘導することに成功し、その発症機序を解析した。陽性荷電化単量体ovalbumin(C-OA,42kDa,等電点10以上)を用い、Balb/cマウスの尾静脈から1mg/300μLを注入し,4時間後、家兎抗OA IgGの3mg/300μLを注入した。c-OAは単独投与ではTBMとボーマン嚢基底膜に24時間まで定着し、免疫電顕では尿細管上皮内ライソソームならびにTBMのlamina densaの内外側のlamina raraに分布した。さらに家兎抗OAIgGの追加投与によりin situ型免疫複合体を形成させ、96時間までその定着が延長した。^<125>Iラベルc-OAならびに^<131>Iラベル家兎抗OAIgGを用いた定量的解析も同様な結果であった。TINの誘導には、同様な処置のマウスを家兎IgGにてアジュバントともに前免疫した。家兎IgGの前免疫群において、c-OAの投与後8日目において、間質内へのマクロファージ、Tリンパ球(CD4<CD8)浸潤が顕著となった。同時期に間質細胞はαSMA陽性、尿細管基底膜之傍尿細管毛細血管にMHC class II分子とICAM-1が発現した。対照群において病変はなかった。以上、低分子量で高度の陽性荷電化外来蛋白(c-OA)は、全身循環を経由してTBMとボーマン嚢に選択的に結合し,さらに液性免疫関与によりin situ型免疫複合体の形成しICAM-1とMHC class IIの発現を介してTINを誘導した。平成11年度は、このモデルにおいて、起炎因子の発現、尿細管上皮や間質構成細胞の形質転換、マクロファージ誘導因子の発現、間質病変の遷延化について、経時的、体系的かつ定量的に解析する予定である。
