| Research Abstract |
平成10年度に引き続き、心筋緻密層形成不全(INCVM)における遺伝子検索を行った。X染色体上の遺伝子異常(Xq28,G4.5)に着目し、本邦の全国調査で診断された症例を基に、新しい疾患概念としてのINCVMの遺伝的背景と遺伝子異常の検索を引き続き行った。我々が実施した全国調査では、小児循環器科を有する16施設より27例のINCVMが報告され、その臨床像と遺伝学的背景についてはすでに報告した(J Am Coll Cardiol 1999;34:233-40)。約40%に家族歴が認められ、4家系に濃厚な家族内集積が認められた。これまでの欧米の報告と異なり、家族歴を有する患者の約半数は女性で、優性遺伝形式の関与も疑われ、この疾患の遺伝形式の多様性が判明した。全国調査により報告された27例の患者において、末梢血リンパ球を採取し、EBウィルスを用い細胞株を作成した。現在、これら株化細胞からDNA遺伝子を抽出し、G4.5遺伝子異常を検索した。遺伝子解析には、主にPCR-SSCP(Single strand comformation polymorphism)analysisとDNAsequencingの方法を用いた。その結果、1例に正常多型が認められた他は、G4.5遺伝子異常は認められなかった。心筋緻密層形成不全には遺伝的多様性があり、これまでに報告されたX染色体上の遺伝子異常であるG4.5以外にも責任遺伝子が存在することが示唆された。
