造影MR血管撮影の最適化に関する基礎的および臨床的研究

Research Abstract

1. MR血管撮影(MRA)の撮影を1.5-Tの臨床用MR装置と4.7-Tの実験用MR装置を用いておこなった。
2. プラスチックチューブを連結させた様々な形状の血管モデルを試作し、この内部に定常流、拍動流および乱流が通過する実験系を確立した。
3. 血管モデルをMR装置の中に設置して流速と流量を任意に変化させ.tine-of-flight法、phase contrast法およびガドリニウム造影剤を使用することによって、最適なMRA像の得られふパルス系列(繰り返し時間(TR)、エコー時間(TE)、フリップ角、フロー感度など)と造影剤の注入方法を検討した。
4. 実験動物を用いて、全身各部位の血管系(動脈および静脈)についてtime-of-flight法、phase contrast法およびガドリニウム造影剤を使用し、最適なMRA像の得られるパルス系列(TR、TE、フリップ角、フロー感度など)と造影剤の注入方法を検討した。
5. 血管モデルにおいて種々の程度の狭窄部位を作成し.time-of-flight法、phase contrast法およびガドリニウム造影剤を使用することにより、狭窄部位を最も正確に描出しうるMRAのパルス系列(TR、TE、フリップ角、フロー感度など)と造影剤の注入方法を検討した。
6. 実験動物において種々の程度の狭窄部位を人為的に作成し、time-of-flight法、phase contrast法およびガドリニウム造影剤を使用することにより、狭窄部位を最も正確に描出しうるMRAのパルス系列(TR、TE、フリップ角、フロー感度など)と造影剤の注入方法を検討した。
7. これらの実験結果を参考にしながら、ボラティアや様々な血管病変を持つ症例に対してMRAを施行し、全身各部位の動脈および静脈について最適なMRAが得られる撮影条件を検討した。
