T細胞抗原受容体CDR3解析による造血細胞移植後免疫再構築のモニタリング

| Research Abstract |
同種骨髄移植および同種末梢血幹細胞移植を受けた症例の末梢血リンパ球からRNAを抽出し、cDNA合成後Vβ鎖およびVδ鎖サブファミリーに特異的なプライマーとCβ1およびCδに対するプライマーを用いたPCRを行い、ついで蛍光ラベルしたCβあるいはCδプライマー(PCR増幅に用いたプライマーより5'上流域に設定)で伸張反応を行った.得られた試料について自動DNAシークエンサー(ABI 377)によりサイズ・スペクトラタイピングを行った.
日和見感染症の多い骨髄移植後日数ヶ月間におけるVβ鎖およびVδ鎖のCDR3の多様性について、臨床所見と合わせ検討した.
(1)ウイルス性間質性肺炎を合併した症例では多数のVβ鎖サブファミリーの欠落が観察された。間質性肺炎を合併しなかった症例においてはVβ鎖サブファミリーの欠落は軽度か全くみられなかった。
(2)一方、Vδ鎖レパートリーについては日和見感染症の有無にかかわらず回復は速やかであった。
結論:骨髄移植後の免疫能はT細胞抗原受容体Vβ鎖の多様性と相関があるが、Vδ鎖の多様性との間には相関はみられなかった。T細胞抗原受容体Vβ鎖CDR3解析は移植後の免疫再構築のモニタリングに有用であると考えられた。
