Research Abstract	1.目的:胃がんの疫学像は、Diffuse Type(DT)とIntestinal Type(IT)で異なっているため、胃がんのリスクファクターが組織型別に異なっているか否かを症例対照研究によって明らかにする。 2.方法:組織学的に胃がんと確認された症例を症例群、症例と性、年令、病院をマッチしたがん以外の患者を対照群とした。生活習慣、食生活の情報は、質問表を用いて直接面接法によって得た。採決血清は、日本でH.PyloriとPrpsinogenについて測定した。解析はCoxの比例ハザードモデルを用いて、オッズ比(OR)を算出した。 3.結果: 《グアテマラ》1999年末までに胃がん例200例、対照例245例について面接調査が実施された。同年7月までのDT79組、IT107組、計186組について解析した。《コスタリカ》1999年末までに胃がん症例250例、対照症例259例について調査が実施された。そのうち同年7月までのDT74組、IT110組の計184組について検討した。主な結果は(1)冷蔵庫使用はDT、IT胃がんともにリスクを低める(グアテマラ)。しかし、コスタリカではリスクの上昇がみられた。(2)IT胃がんがDT胃がんより食品に関連が強い傾向が見られる。(3)DT、IT胃がんともH.Pylori感染がリスクを上げている結果は得られなかった。(4)Pepsinogen PGI/II比による萎縮性胃炎陽性者はDT、IT胃がんともにリスクを上げているが、コスタリカがより顕著である。今後、最終の症例収集を待って、最終解析を実施し、各分担分野に応じて、発表論文を作成する。
