口腔癌の自己増殖促進因子及び血管新生因子遺伝子を標的とした遺伝子診断・治療の研究

Research Abstract

ヒト口腔前癌病変におけるVEGF.FGF-2およびHBp17/FGF-BPの発現:HBp17蛋白に対するMoAb(IgG1)C9を作成し、正常口腔粘膜上皮、口腔前癌病変、口腔扁平上皮癌組織における、VEGF.FGF-2およびHBp17/FGF-BPの発現を免疫組織学的に検討した。その結果、正常口腔粘膜扁平上皮においては、発現強度はいずれも非常に弱いもののHBP17とFGF-2は有棘細胞層においてVEGFは上皮全層において発現していた。一方、細胞異形に伴いHBp17、FGF-2およびVEGFの上皮組織における発現強度は増加した。特にHBp17およびFGF-2は基底膜側細胞での強い発現を認めた。過形成症、異形成症および口腔扁平上皮癌のなかで高度異形成症においてHBp17,FGF-2およびVEGFは最も高い発現を示した。また,フォンウィーブランド因子の発現を指標に検討した血管新生も高度異形成症において最高値(19.28±6.38)を示した。いずれの症例においてもHBp17とFGF-2の局在は一致した。各症例においてHBp17,FGF-2とVEGFの発現はそれぞ正の相関(P=0.04)を示した。また,FGF-2,HBp17とVEGFの発現強度とMVDも正の相関(P=0.04)を示した。以上の結果から,HBp17が口腔粘膜上皮の癌化および血管新生に重要な働きをしていることが示され,口腔癌の診断・治療の分子標的としての有用性が示唆された。さらに,基底膜浸潤(破壊)に先んじて、高度異形成症で最も高い血管新生を示すことが明らかとなり,同時期における血管新生をターゲットにした治療の有用性が示唆された。口腔扁平上皮癌におけるFGFR3遺伝子の変異とその機能解析:正常上皮細胞の増殖はFGF-1,FGF-2,FGF-7/KGFにより促進されたが、扁平上皮癌細胞の増殖はこれらFGFsに依存しないことを明らかにした。そこでこれら細胞におけるFGFR遺伝子群の発現を検討したところ、扁平上皮癌細胞は正常上皮細胞と同じFGFR遺伝子群を発現していることが明らかとなった。そこでSCC細胞ではFGFRの機能異常が伴う可能性が示唆されたため、SCC細胞および口腔SCCにおけるFGFR遺伝子の変異をPCR-SSCPおよび(direct sequence法にて検討した。その結果、口腔扁平上皮癌71症例中44症例にFGFR3遺伝子のチロシンキナーゼ領域をコードする2128番目の塩基グアニンのチミンへの変異を認め、同変異は697番目コドンであるアミノ酸グリシンのシステインへの置換を示唆した。口腔扁平上皮癌71症例中約61%の43症例に(i2128T変異に基づくG697C置換を認めた。そこで、変異型FGFR3の機能を明らかにするため、バキュロウイルス発現系を用いて変異型FGFR3タンパクを発現しそのチロシンリン酸化活性を検討したところ、野生型と比べて変異型FGFR3ではFGFsに依存しない自己リン酸化活性の著しい亢進を認めた。従って口腔扁平上皮癌においては高い頻度でFGFR3チロシンキナーゼ領域の変異が存在することが明らかとなり、口腔扁平上皮の癌化過程にFGFR3遺伝子異常が深く関与していることが明らかとなるとともに、FGFR3遺伝子を標的とした遺伝子診断・治療の可能性が示唆された。
