血栓性血管病変進展におけるケモカインと血小板マイクロパーティクルに関する研究

| Research Abstract |
1.血栓性疾患におけるマイクロパーティクル(PMP)・ケモカイン・サイトカインと可溶性接着分子測定:各種血栓性疾患の患者血漿を用いて、活性化血小板、PMP、サイトカイン、ケモカインおよび可溶性接着分子を測定した。特に血管病変の程度の差によるPMPの関与について検討するため、糖尿病、閉塞性動脈硬化症(ASO)および急性心筋梗塞症例を対象として測定した。ケモカインのうち、MCP-1とRANTESは糖尿病症例において増加しており、いずれも活性化血小板と正の相関関係を示した。一部のASO患者では、炎症性サイトカイン(IL-6、IL-1β、THFα)の増加が観察された。急性心筋梗塞症例でも炎症性サイトカインの増加が観察され、同時にG-CSFやTPOなどの造血性のサイトカインの増加も認められた。いずれの疾患群においても血管病変の進行にしたがい、可溶性接着分子(sVCAM-1、sICAM-1、sP-selectin、sE-selectin)の増加が観察され、活性化血小板やPMPとの相関性を示す症例もみられた。これらの結果より、PMPは、炎症性疾患における血管病変の進展に関与していることが示唆された。
2.糖尿病、閉塞性動脈硬化症、急性心筋梗塞患者における抗血小板療法の影響:上記の各種血栓性疾患に対して抗血小板療法(チクロピジン、シロスタゾール)を実施した。血小板活性化マーカーやPMP値の改善に伴い、ケモカイン・サイトカイン・可溶性接着分子の正常化が観察された。適切な抗血小板療法は、PMPが関与する血管病変の進展を改善しうると考えられた。
