サイトメガロウイルス臨床分離株の病原性ならびに治療抵抗性に関する分子疫学的研究

サイトメガロウイルス(CMV)の病原性の発現には宿主側の要因が重要であるが、本邦における小児科領域よりのCMVの臨床分離株の特性に関しては十分に検討されていないのが現状である。また各株間における病原性および治療抵抗性の差異に関しても不明な点が少なくない。今回、我々はPolymerase chain reaction (PCR)-Rrestriction fragment length polymorphism (RFLP)法で臨床分離株よりCMV major immediate early (MIE)領域のDNAの増幅を行い、制限酵素Hae IIIおよびHinf Iを用いて切断点を比較した。同様にglycoprotein B(gB)領域DNAの増幅も行い、Rsa IおよびHinf Iを用いて切断点を比較を試みた。札幌市内のN病院およびS病院産科において分娩した妊婦より出生した新生児の尿より組織培養にてCMVの分離を試みた。ガンシクロビル、フォスカーネットなどの抗ウイルス薬剤に耐性を有すると推定される臨床分離株は採取できなかった。臨床分離株は、ある遺伝子領域では各制限酵素による切断パターンに著明な差異は認められ、数領域の遺伝子について解析を行った。CMVの糖蛋白にはglycoprotein B(gB)、gH、gL、gM、IMP、gp65、gp48、gp47-52、GCR33などが知られている。主要なエンベロープ蛋白であるgB(UL55産物)は中和抗体の主な標的となる。感染初期にgBはウイルスの吸着およびエンベロープと宿主細胞の細胞膜との融合に関与し、gHも同様に膜融合に関与する。これらの中でgB領域の切断パターンの比較では各臨床分離株の病原性および免疫原性の解析に応用が期待できた。gB遺伝子には4型の存在が知られており、AIDSなどのの免疫抑制状態における病原性の差異が報告されているが、今回の研究ではそのうちの三つの型の存在が明らかになった。
