Research Abstract |
To determine whether atrophic changes occur in the ipsilateral cerebral peduncle and the substantia nigra in Parkinson's disease patients who have undergone posteroventral pallidotomy (PVP), we obtained routine magnetic resonance (MR) images (T1- and T2-weighted images) of 17 patients with Parkinson's disease who had undergone unilateral PVP and measured the width of bilateral cerebral peduncle at the mid-point of the inner margin of it. Then, we assessed the topographical alteration of the substantia nigra in six patients more than 1 year after PVP by multishot diffusion-weighted imaging, which was a new imaging method to depict the substantia nigra. In MR images within 1 year after PVP, a comparison of the ipsilateral side/contralateral side ratio of the cerebral peduncle of patients who had undergone PVP and that (unaffected side/affected side ratio) of the preoperative images revealed no significant difference (P>0.05). In MR images from 1 to 2 years after PVP, there was a significant difference in the ratio of the cerebral peduncle (P<0.01). A significant difference was still evident in MR images more than 2 years after PVP (P<0.001). Multishot diffusion-weighted imaging revealed the shrinkage in the posterolateral part of the substantia nigra in the ipsilateral cerebral peduncle after PVP in all six patients. In the chronic stages following PVP, the ipsilateral cerebral peduncle is reduced in size. This midbrain atrophy should reflect the shrinkage of the posterolateral part of the substantia nigra.
