胆道・膵領域癌における分子生物学的診断に基づく治療体系の確立に関する基礎的研究

Research Abstract

本研究は、膵・胆道領域癌の切除症例におけるprospective studyであるため、現時点では標本・サンプルの採取を行い、保存している段階である。具体的には、現在までに、膵癌2例、下部胆管癌1例、十二指腸乳頭部癌3例、膵粘液性嚢胞腺腫1例の術前末梢血、術中門脈血、腫瘍、腹腔神経節、膵頭神経叢、上腸間膜動脈周囲神経叢、リンパ節の採取・保存を終了した。そのうち、腫瘍、腹腔神経節、膵頭神経叢、上腸間膜動脈周囲神経叢、リンパ節においては、ヘマトキシリンエオジン染色にて病理学的診断を行った。今後は更にサイトケラチン免疫組織化学染色を行う予定である。また、それぞれのサンプルにおいてDNAを抽出し、nested PCRを応用したアレル特異的増幅法(nested MASA法)にてK-ras遺伝子突然変異を検索し、分子生物学的診断を行う予定としている。
さらに本研究の予備実験として、当科における過去の膵癌切除症例のK-ras遺伝子突然変異の検索を行っている。現在までに、膵癌切除症例72例の腫瘍部のDNAをホルマリンブロックより抽出した。このうち、41例にnested MASA法を施行し、31例(75.6%)にK-ras遺伝子突然変異を認めるという結果を得た。今後は、残りの31例のK-ras遺伝子突然変異の検索を行い、変異陽性例においてはさらにリンパ節でのK-ras遺伝子突然変異の検索を行う予定としている。
