Research Abstract

Oral examinations were performed periodically in 8 patients(6 : Nephrotic Syndrome, 2 : chronic renal failure)to investigate the onset mechanism of calcium channel antagonist Nifedipine-induced gingival overgrowth(GO)in children. Patients consisted of 3 boys and 5 girls, age 7-20 yr, who had been prescribed Nifedipine and/or Amlodipine and/or Cyclosporin. GO was present in 5 patients. Oral examination data comprised the erupted teeth, probing depth and plaque score. Image capture was performed with a digital camera Fuji DS-560^R fitted with a Medical Nikkor 120 mm telephoto lens. We used a quantitative method with digital image analysis processing of images with a computer and with the software package Adobe Photoshop 5.0 to evaluate the degree of gingival overgrowth.
We selected the dental areas not covered by the gingiva to quantify in square pixels of the upper and lower anterior regions and from the change of the calculated pixels we diagnosed if GO was increasing or decreasing.
In one case, the dental area's pixels decreased and it showed the increased gingival overgrowth. However, in the other cases the changes of pixels were not significant. Then the images taken at the different time were piled on. It revealed the change of the gingival shape, the overgrowth in the papillary region and the recession in the middle of the dental crown.
In one patient, lower lateral incisors were erupting and the GO was observed, however, there was no GO in the primary dentition. Three cases without GO had been prescribed Cyclosporin after the eruption of permanent teeth. This shows tooth eruption may be responsible for the onset of GO.
