強皮症属変形成における各種サイトカインの関与について

Research Abstract

汎発性強皮症(systemic sclerosis;SSc)は原因不明の結合織疾患であり、コラーゲンをはじめとする細胞外マトリックスの皮膚やその他の諸臓器への過剰な沈着を特徴とする。また、SScでは多彩な免疫な免疫学的異常が認められ、血管障害、細胞外マトリックスの過剰と相まってその病態を形成している。TGF-βで発現が誘導される、tissue inhibitor of metalloproteinase type 1(TIMP-1)はその自体細胞増殖活性を有し、SSc病変成への関与を報告している。最近我々はTIMP-2についても病変形成に関係を見出した。汎発性強皮症は自己免疫性疾患であるが、皮膚、臓器の線維化に対し副腎皮質ステロイド剤が有効でないことが古くから指摘されている。近年、他の自己免役性疾患でその有効性が確認されつつある各種免疫抑制剤も、まだ試用中だがやはり無効である。γinterferonnがその免疫調節作用とコラーゲン合成抑制作用の画面から有効性が期待されているが、現実の有効性については期待ほどではない。現状において汎発性強皮症研究の中心は病変部由来の繊維芽細胞の特質を明らかにすることにあり、これら難病の発症メカニズムの解明、治療法の開発のステップとして重要と考える。
