強皮症病変形成における各種サイトカインの関与について

| Research Abstract |
汎発性強皮症(systemic sclerosis;SSc)は原因不明の結合織疾患であり、コラーゲンをはじめとする細胞外マトリックスの皮膚やその他の諸臓器への過剰な沈着を特徴とする。また、SScでは多彩な免疫学的異常が認められ、血管障害、細胞外マトリックスの過剰と相まってその病態を形成している。TGF-βで発現が誘導される、tissue inhibitor of metalloproteinase type1(TIMP-1)はそれ自体細胞増殖活性を有し、SSc病変成への関与を報告している。最近我々はTIMP-2についても病変形成に関係を見出した。現状において汎発性強皮症研究の中心は病変部由来の線維芽細胞の特質を明らかにすることにあり、これら難病の発症メカニズムの解明、治療法の開発のステップとして重要と考える。さらに我々はSSc患者血清では血管新生サイトカインであるbasic fibroblast growth factor(bFGF)やvascular endothelial cell growth factor(VEGF)の濃度の上昇が認められることを報告した。また臓器特異的炎症性疾患である炎症性腸疾患の腸、皮膚の罹患部における血管新生サイトカインの発現も検討した。これらの疾患の病勢とサイトカイン発現には相関があり、今後は病変形成への関与が考えられる。
