移植レシピエント肝からの未知の劇症肝炎ウィルス遺伝子のクローニング

| Research Abstract |
我が国の劇症肝炎の原因は従来B型肝炎ウィルス(HBV)によるものが多かったが、最近ではそれに代わって原因不明の劇症肝炎が増加してきている。こうした例では肝移植後再発する例が多くみられることから未知のウィルスの関与が想定されている。そこで本研究では劇症肝炎の原因と考えられる未知の肝炎ウィルスを同定するために、生体肝移植手術を受けた劇症肝炎患者の肝から、differential display法を用いて、原因となる新規肝炎ウィルス遺伝子をクローニングすることを試みた。
肝移植時に得られた原因不明の劇症肝炎患者の肝とドナーの肝からRNAを抽出し、RT-PCRにて増幅した。これらを用いてdifferential displyをおこない、劇症肝炎患者の肝で明らかに発現が亢進している遺伝子産物を特定した。また肝炎の劇症期と快復期の血清を用いて同様の検討を行った。こうして得られた遺伝子36個(肝組織32遺伝子、血清4遺伝子)について、核酸配列をデータ・バンクで調査したところ、その大半がNF-κB,IKK-β,NIK,traf-2,traf-2,bad,Bcl-2,HGF,TNF-αなど、アポトーシス、アンチアポトーシスに関連した遺伝子や増殖因子であった。実際にこれらのプローベを用いてIn situ hybridizationやノーザーンブロット解析をおこなったところ、これらすべての遺伝子の発現が増強していた。一方、発現が増強していた遺伝子の中で未知の遺伝子が2個あったが、これらの配列を解析したところいずれもウィルス遺伝子ではなかった。その後10例の劇症肝炎患者について解析を継続したが、その中で発現が増強している遺伝子がさらに40個得られた。現在これらの遺伝子についてその核酸配列を調べて、新しい肝炎ウィルス遺伝子の同定を進めていきたい。
