| Research Abstract |
本年度の研究実績
1.記述疫学的・臨床疫学的研究
アジア・南米などの環太平洋地域の調査では、性、がんの部位、組織型、進行度等を勘案してもEBウイルス関連胃がんは北米・南米に多く、アジア各国で少ないことが明らかになったことから、生活習慣、民族的背景との関連が示唆された。一方、ペルーやパプアニューギニアではEBウイルス関連胃がんの割合が非常に低く、さらに検討を進める必要がある。
2.分析疫学的検討
鹿児島県での面接調査の結果や各国の病理学的知見などから、胃底腺領域粘膜の機械的障害がEBウイルス関連胃がんの発症と関係している可能性が示唆された。今年度末までにコロンビアで約500例の胃がん症例の面接調査を完了する予定で、さらに詳しい検討を進める。
3.ウイルス学的検討
チリとコロンビアで得られたEBウイルス関連胃がんと健常者うがい液の試料を検討した結果、EBVのXhol周辺領域における遺伝子多型の重要性が示唆された。
4.臨床病理学的検討
細胞周期制御に関与するp16蛋白と細胞接着因子E-カドヘリンとその裏打ち蛋白であるβ-カテニンの発現パターンを免疫組織化学染色法で調べた。EBウイルス関連胃がんでは、p16蛋白発現抑制が顕著であり、宿主DNAのhypermethylationが示唆された。p16蛋白発現と予後との間に関連は認めなかった。EBウイルス関連胃がんとEBER陰性胃がんでは、E-カドヘリンの発現に顕著な差は認めなかったものの、EBウイルス関連胃がんでβ-カテニンの発現が比較的保たれている傾向を認めた。
