| Research Abstract |
本研究は大腸前がん病変である大腸腺腫と関連する生活習慣要因を明らかにするための断面的疫学研究である。自衛隊福岡病院および熊本病院において定年退職前健康診断を受診した男性自衛官者を対象にして、喫煙、飲酒、運動および食物摂取などの生活習慣要因に関する調査をおこない、大腸内視鏡検査をはじめとする医学的検査結果を系統的に収集整理した。また、ほとんどの調査対象者から採血し、血漿と白血球成分を凍結保存している。平成12年度からの累積の調査例数は合計で2300例余りに達した。
以前からのデータを含め約2500名を対象として、糖尿病と大腸線腫の関係を部位別に検討し、糖尿病が右側大腸線腫リスクの高まりと関連していることを明確にした。また、616名について、血漿インスリン様増殖因子IGF-Iと結合蛋白IGFBP-3を測定し、IGF-Iが大腸線腫と関連していることを観察した。さらに、肥満度がIGF-IおよびIGFBP-3と強く関連しており、欧米での報告と異なる結果が得られた。
また、この研究で用いている食物摂取調査票の妥当性を1年間28日食事記録にもとづく推定摂取量を用いて検討した。脂肪摂取量を除き、ほとんどの食物・栄養素について良好な妥当性を確認した。食物・栄養要因と大腸線腫については詳細な解析を進めている。
