Research Abstract	結核体質:抗酸菌易感染例にIFN-γ受容体など抗酸菌排除に重要であるTh1経路のサイトカイン/受容体遺伝子の異常が報告されている。今回我々は播種性BCG副反応を呈した10症例で、一般免疫能検査、Th1サイトカイン産生・及びIFN-γ受容体1遺伝子の変異を検討し、3症例とその父親1例においてIFN-γ受容体1遺伝子の変異を見出した。症例1の親子例、症例2に翻訳開始地点より818番目に、症例3は811番目に4塩基欠損をヘテロで検出した。播種性BCG副反応例にIFN-γ受容体1遺伝子変異が3家系4症例に見出され、我が国の結核体質の一つであることを明らかにした。アレルギー体質:今回、Th2サイトカインを抑制するとされるIFN-γ、およびIFN-γのシグナル伝達に関わる関連遺伝子群、CD28/CTLA-4、ヒスタミンH1・H2レセプターとアトピー喘息の関連を検討した。CD28/CTLA-4、ヒスタミンH1・H2レセプターとアトピー喘息の関連は見られなかったがIFN-γイントロン1およびIRF-1の遺伝子多型がアトピー喘息と関連した。IFN-γレセプタ-1/2の遺伝子多型は有意な関連は示さなかった。今回我々は、IFN-γ、あるいはそのシグナル伝達に関連する遺伝子群の多型の組み合わせとアトピー喘息との間に明確な関連性を見出した。このことは、アトピー喘息の発症機序におけるTh1サイトカインの重要性を意義づけるとともに、Th1反応の誘導によるアトピー治療の可能性を示唆する結果と考えられる。
