重症型魚鱗癬の発症における、角層のセル・エンベロープ形成異常の果たす役割の解明

Research Abstract

(1)免疫電顕による解析
前年度集積された患者の内、共焦点レーザー顕微鏡による観察等でcornified cell envelopeの形成異常が確認された症例に対して、前年度に引き続きcornified cell envelopeの前駆蛋白に関して、免疫電顕により微細構造レベルでの発現分布状態を解析し、患者病変部表皮における分子レベルでのcornified cell envelopeの形成異常を定量的に明らかにした。
(2)遺伝子変異の同定
十分なインフォームドコンセントに基づき、患者ならびに家族から採血し、genomic DNAを抽出した。前年度の検索、および、上記(1)の結果より、発現、分布に異常のみられた蛋白についてHeteroduplex法、direct sequencing法等の方法を用いて、遺伝子変異を解析した。
(3)病因蛋白、遺伝子変異と魚鱗癬病像の相関関係の解明
前年度および上記の結果を総括的に解析して、葉状魚鱗癬、非水疱型先天性魚鱗癬様紅皮症における遺伝型・表現型の相関関係(genotype/phenotype correlation)を明らかにした。各症例における原因遺伝子変異(遺伝型)と皮膚における表現型(臨床症状、発症年齢、重症度、予後、組織における微細構造変化、蛋白発現状態の変化、免疫電顕レベルでの原因分子の微細局在様式の変化)との関連を総括的に解析し、本疾患の遺伝型・表現型の相関関係を明らかとした。
