微小転移制御をめざした癌外科治療法の確立

Research Abstract

前年度までに、各種固形癌に対して確立した遺伝子微小転移検出法を臨床応用することを目的に研究をおこなった。
1.術中迅速遺伝子診断に基づく食道癌のリンパ節郭清範囲決定のProspective study行った(現在までに73例に施行)。食道癌根治手術症例において、縦隔最上部に位置する両側反回神経周囲リンパ節を迅速遺伝子診断と従来の病理学的検索とを用いて術中診断し、頸部リンパ節転移の予測を行ない、郭清範囲を決定するprojectである。この結果、術中迅速遺伝子微小転移診断を導入することにより、頸部リンパ節郭清の適応決定が、より正確に行える可能性が示唆された(論文投稿中)。
2.胃癌の腹膜再発に対する治療戦略として、術前の腹腔鏡検査時の腹腔洗浄液を用いた定量的遺伝子診断の(1)術前腹腔内化学療法の治療効果、(2)術後の腹膜再発予測への臨床応用を検討中であり、現在16例に施行し、遺伝子診断の有用性が示唆されている。今後、迅速遺伝子診断のさらなる改良(より短時間で、より簡略な方法で)を行い、術中迅速遺伝子診断の臨床応用の幅を広げるとともに、Molecular stagingによる、high risk症例の抽出とそれに対する治療戦略を確立していく予定である。
