Research Abstract

現在本邦で行われている標準的胃癌手術は胃の広範な切除と盲目的リンパ節郭清であるが、これは一律な根治性が期待できる一方で機能障害を招き、根治性と機能性の追及という二律背反をわれわれに課している。この問題を解決するには、手術中に的確なリンパ節転移診断を可能とする手法により、過不足のない手術を実現することである。癌に特異的に反応する抗体を術前に投与し、これをガイドとしつつ、癌病巣のみを選択的に摘出する手術が完成すれば、不必要で過度の浸襲を避けることができる。
1)申請者は胃癌に特異的に反応するモノクローナル抗体をlimiting dilution法でクローニングし、より選択性の高い抗体を作成した。
2)新たに作成した抗体を精製し、抗体の異種性を軽減するために抗体にポリエチレングリコール(PEG)を結合させた。抗体/PEG比は1:2であった。
3)抗体活性を計測した結果、PEG化抗体は元の抗体の結合活性の90%を保持していた。
4)PEG化抗体に125-Iを標識し、これを担癌ヌードマウスに投与し、体外からの検出が可能かどうかとその精度を計測した結果、人に応用するのに十分であることが判明した。
