Research Abstract |
We have developed an innovative device which enables to per fuse ,the coronary artery during the off-pump coronary artery bypass grafting. The system is composed of an inlet cannulae from the femoral artery, an outlet cannular to the coronary artery, a one way valve, and a reciprocal syringe pump to per fuse blood synchronously during the diastolic phase of the heart. We examined the clinical efficacy of this system this year with 200 consecutive cases in a clinical setting. There were no operative deaths. Only one case of perioperative infarction was observed, and the graft patency was 99.1% at the site of LITA-LAD anastomosis. There were no ECG changes, and no significant changes of systemic blood pressure, pulmonary artery blood pressure, heart rate, or serum Toroponin T. Sufficient adequate per fusion rate was detected at 0.1 ml/beat, and the adequate per fusion timing was 40% R-R interval during the diastolic phase. As the practice of the off-pump CABG further becomes common, the clinical efficacy of this system will no doubt increase in the near future.
