細径顎関節鏡の開発による顎関節下関節腔鏡視診断の確立

Research Abstract	実験的研究;細径関節鏡の診断応用の臨床的研究の事前準備とし以下の2つの研究を行った。(1)ヒト関節円板表層の形態変化に付随した構造タンパクの変化を知る目的で、解剖体より得たヒト関節円板組織を用いたII型プロコラーゲンC末端ペプチドに対する免疫組織学的研究を行った。(Kondoh T, et al. 2003 Arch Oral Biol 48, 621-625)(2)細径関節鏡の診断精度を検討する目的で、ウサギ膝関節に作成した変形性関節症モデルを用いた実験的研究を行った。この結果から細径関節鏡の診断精度は従来の関節鏡に比し良好な感度と特異度を示した。(後藤美香,近藤壽郎 2004 日口外誌50,3号掲載予定) 臨床的研究;細径関節鏡による顎関節腔内病変の診断を目的とし以下の3つの研究を行った。(3)われわれが開発した細径関節鏡を使用し顎関節内障と変形性関節症の患者の上関節腔の内視検査を行い、臨床症状と各種画像検査所見との関連性について検討した。(Kondoh T, et al. 2003 Oral Surg Oral Med Oral Pathol 95, 544-551. Hamada Y, Kondoh T, et al. 2003 J Oral Maxillofac Surg 61, 437-441)当初目的とした顎関節関節腔用微細径関節鏡の開発、その検査特性の検討ならびに臨床応用の基礎的検討を完了した。今回われわれが開発した微細径関節鏡は従来にない低侵襲関節鏡検査を可能とする性能を有する器機であり、その診断精度、検査特性は従来の関節鏡と同等であった。また臨床応用においても低侵襲鏡視を可能とし、安全性を証明した。
