| Research Abstract |
老人性肺炎は難治性であり、肺炎での死亡率は30年前と不変である。
私共は老人性肺炎は不顕性誤嚥で生じることから、抗生剤の投与と同時に不顕性誤嚥を予防するACE阻害剤とアマンタジンを同時に併用することにより、老人性肺炎の治療法の改善につながるか否かを調べた。結果は抗生剤のみ使用した肺炎群に比べてACE阻害剤とアマンタジンを抗生剤と同時に使用した肺炎群では、抗生剤の使用量が半分で済んでいた。また、入院日数、肺炎での死亡率、MRSAの出現を約2/3に減少できた。
老人性肺炎が難治性であるのは抗生剤で治療しようとする力と、同時に生じる不顕性誤嚥により悪化させようちする力とがせめぎ合っているからで、不顕性誤嚥を除くことにより、若い人の肺炎と同等に近い治療成績が得られた。従来、老人性肺炎の予防について報告してきたが、治療についても画期的方法と言える。
