| Research Abstract |
重症の難治性眼表面疾患に対してはこれまで、羊膜移植や培養角膜上皮移植など様々な外科的治療法が開発され、それなりの成功をおさめてきた。しかしながら難治性眼表面疾患の多くは両眼性であり、これらの治療法は他人(アロ)からの組織の移植に頼らざるをえず、術後の拒絶反応などの問題が術後成績に大きく影響を与えているのが現状である。今回我々は、自己(オート)の粘膜上皮を用いた移植法の開発を念頭に、羊膜上での培養口腔粘膜上皮シートの作成を検討した。白色家兎の口腔粘膜組織から上皮細胞を採取し、保存羊膜を基質としてcell suspension法により3週間培養した。これらの過程においては、3T3線維芽細胞との共培養、上皮の分化誘導を施すair-lifting法を併用した。作成した培養口腔粘膜上皮シートのケラチンの発現、およびin vivoでの移植によりその透明性維持を検討した。
羊膜上に培養された口腔粘膜上皮はケラチン3,4,13を発現し、角膜上皮のケラチン発現に近似していた。さらにin vivoでの移植では口腔粘膜上皮は眼表面で長期にわたりその透明性を維持した。
これらの結果により、口腔粘膜シートは角膜上皮の代用となる新しい上皮組織となる可能性が示唆された。
