免疫不全状態におけるウイルス感染症の再活性化の病態解明と予防・治療

Research Abstract

平成13年度の研究成果は以下の通りである。
1)蛍光プローブを用いたreal-timePCR法によるウイルス定量システムを構築した。既に確立/報告しているherpes simplex virus、varicella-zoster virus、cytomegalovirus、Epstein-Barr virus(EBV)に加え、human herpesvirus(HHV)-6、HHV-7、HHV-8についても新たに定量システムを確立した(論文発表)。
2)ウイルス特異的細胞性免疫を迅速に定量するために、intracellular cytokine assayを用い、ウイルス抗原に対しinterferonγを放出するCD8+細胞をFACSで測定するシステムを構築した(論文発表)。さらに近年独自に開発したtetrameric HLA-A^*2402-peptide complex(テトラマー)を用い、抗原特異的CD8^+T細胞をより迅速・簡便に測定し比較した(論文発表)。
3)上記の方法を用い、造血幹細胞移植患者でvirus loadおよびウイルス特異的細胞性免疫を測定し、ウイルス感染症の迅速診断を行うとともに、これら感染症が重症化するメカニズムを解析した(論文準備中)。
4)造血幹細胞移植時の重症合併症であるEBV関連リンパ増殖性疾患の診断に、real-timePCR法によるウイルス定量システムが有用であること、治療法の選択にウイルス特異的細胞性免疫の定量測定が役立つことも明らかにした。またEBV関連リンパ増殖性疾患の発症因子としてanti-thymocyte globulinの使用が重要であることを示した(論文発表)。
