Research Abstract

初年度は、T細胞の機能分子であるperforinとFasの異常による原発性血球貧食リンパ組織球症(primary HLH)と自己免疫性リンパ増殖症候群(ALPS)の解析を行った。これらは、各々の分子異常によりactivation-induced cell death(AICD)が障害され、高サイトカイン血症や自己免疫疾患を引き起こす先天性リンパ増殖症である。
1.primary HLH
2歳未満発症15例のperforin遺伝子解析を行い、その蛋白発現をFlow cytometry(FCM)と免疫染色にて検討した。家族歴のある8例中2例(25%)、家族例のない7例中1例(13%)に遺伝子異常(新変異4と既知変異1)を同定した。変異のある生存例のCD8^+細胞とCD56^+細胞にperforin蛋白の発現はなかった。(Br J Haematol,2002 in press)。
2.ALPS
リンパ増殖の著明な2例(Pt1,Pt2)と対照1例(Pt3)のサイトカイン発現を解析した。CD4^-CD8^-(Double Negative : DN)αβT細胞はPt1(19%)とPt2(35%)がPt3(4%)より多く、血清IL-10濃度はPt1(92pg/ml)とPt2(34pg/ml)がPt3(9pg/ml)より高かった。Pt1とPt2の末梢血FCM解析では、CD3分画中のIL-10産生細胞はCD4,CD8細胞より多く、αβT細胞分画のそれはYδT細胞分画より多かった。定量real-timePCRでは、Pt1とPt2のDNT細胞におけるIL-10mRNA発現はCD4,CD8T細胞より約100倍高かった。IFNγとTGF-βにこの傾向はなかった。IL-10mRNAはDNαβ^+細胞に、IFNγmRNAはDNγδ^+細胞に高発現した。ALPSのDNαβT細胞はIL-10を過剰発現し、臨床像に関与することが示唆された。(投稿中)
