| Research Abstract |
晩期閉塞の主要因である吻合部内膜肥厚の成因とその制御について、MCP-1に注目して、その遺伝子導入による内膜肥厚抑制効果を検討した。
【方法】MCP-1と競合作用を示す変異型MCP-1(7ND)遺伝子あるいはPBS(control)をelectroporation法を用いウサギ大腿筋に導入し3日後に頚動脈をバルーン障害した。障害後3、7,28日後にharvestを行い、以下の項目を検討した。
【結果】1.大腿筋へのluciferase遺伝子導入によりその発現をみるとluciferase活性は2日目には7.5×107RLU/mgに達し、14日目にはその1/7まで活性が低下するが28日後も同レベルで、発現が長く持続することが確認された。2.摘出した頚動脈をRAM11にて免染した。全細胞を占めるRAM11陽性細胞数の割合(%)は内膜、中膜それぞれ7ND導入群で有意に低く、MΦの浸潤が7NDにより有意に抑制された。3.障害28日後の内膜面積およびI/M比は7ND群はいずれもcontrol群に比し有意に小さく内膜肥厚が抑制された。4.障害28日後の内腔面積、IEL(内弾性板長)、EEL(外弾性板長)はそれぞれ7ND群はいずれもcontrol群に比し、有意に大きくnegative remodelingが抑制されていた。
【考察】MCP-1の作用をblockすることでバルーン障害後の内膜肥厚およびnegative remodelingが抑制できた。又この遺伝子の導入がelectroporation法により容易にかつ効率よく可能で、その持続期間も長いことから新しい遺伝子治療として期待できると考えられた。
