Research Abstract

The transplantable acellular allogeneic dermal matrix (ADM), which is effective as a scaffold of a tissue engineered skin was developed. By basic studies, following findings were obtained.
1 The preparation of ADM without protease treatment is better for cellular attachment.
2 The sterilization of ADM with high concentration of glycerol is acceptable for improving safety of ADM transplantation.
3 A tissue-engineered skin equivalent based on ADM is homologous to the normal human skin.
4 The cultivating methods without BPE and feeder layers have been established for the regenerative skin surgery.
Based on these findings, I have developed a transplantable tissue-engineered skin equivalent composed of autologous cultured keratinocytes and fibroblasts and decellularized allogeneic dermis. In a 3-week process, cultured autologous keratinocytes from burn patients were incubated with and became firmly bound to decellularized allogeneic dermis obtained from cadavers. The tissue-engineered autologous skin equivalents were transplanted in a one-stage procedure to the excised-third degree burn wounds of 3 patients. The graft survival rates in the 3 patients were 93%, 96%, and 100%, without delayed graft loss or graft fragility.
Conclusion : These results suggest that tissue-engineered skin equivalents based on autologous transformation of decellularized allogeneic dermis can be used for permanent repair of full-thickness skin defects.
