アジアにおけるエキノコックス症、嚢虫症流行拡大に関する免疫・分子疫学調査

Research Abstract

アジアに於ける人畜共通、難治性寄生虫疾患として、エキノコックス症、有鉤嚢虫症に関する疫学調査を推進している。平成14年度の研究実績は下記の通りである。
1.中国におけるエキノコックス症疫学調査では中国四川省を中心とする多包虫症と単包虫症が重複流行地域における集団画像診断、採血(中国研究グループ)、現地調査(日中合同)血清検査(日中合同)、画像病巣の形態的特長から多包虫症と単包虫症とに類別し、病巣の大きさ、年齢、性別、地域性についてそれぞれの成績相互評価(日中合同)を試みた。画像診断と血清診断法の相関は非常に高く(94%)、多包虫症を単包虫その他の疾患から鑑別する非常に有効な検査手段であることが判明した。この血清診断法については迫がEm18の生化学的性状を解析し遺伝子組み換えEm18抗原を作製し、gold-standard serologyと評価される成績を発表し、国際評価が確立している(Sako 他 2002. J Clinical Microbiology 40,2760-2765;Ito 他 2002. JCM 40,4161-4165)。
2.エキノコックス条虫のミトコンドリアDNA全塩基配列の解読に中尾(Nakao 他 2002. Mitochon drion 1,497-509)が成功し、さらに北海道内の多包条虫1個体ごとのmicrosatellite DNA解析から条虫の生殖戦略に関する重要な成果を得た(Nakao 他、投稿中)。
3.有鉤嚢虫症に関してはインド、インドネシア、中国、タイ、ベトナムで調査に当たり、貴重な寄生虫サンプル、血清、糞便の解析が始まった。人体から外科的に摘出された嚢虫、排泄された条虫片節を用いてミトコンドリアDNAの比較解析により(a)有鉤条虫がアジア型とアフリカ・アメリカ型に大別されることを中尾(Nakao et al. 2002. Parasitology 124,657-662)、(b)人体寄生3種テニア科条虫を鑑別できることを山崎(Yamasaki 他 2002. JCM 40,3818-1820)が証明し、糞便内DNA検査法を開発している(未発表)。
4.インドネシアでは北スマトラ島でTaenia asiaticaが流行し、バリ島ではTaenia saginataが流行し、イリアンジャヤではTaenia soliumが流行していることが判明してきた。
平成15年度はバリ、イリアンジャヤ、中国、モンゴルを中心に調査する予定である。
