Research Abstract

無けいれん性電気けいれん療法(m-ECT)は、難治性うつ病等に非常に有効な治療であり臨床的に確立した治療法であるが、その機序には不明な点が多い上研究も遅れている。本研究ではm-ECTの適応となった大うつ病性障害患者4名を対象に、ヒトの脳でm-ECTの前後でのドーパミンD2(D_2)受容体とセロトニン1A(5HT_<1A>)受容体の変化の有無を選択的リガンドである[^<11>C]FLB457と[^<11>C]WAY 100635を用いてpositron emission tomography(PET)によって評価した。受容体結合の評価は小脳を参照部位にしたreference tissue modelによって結合能を求め比較した。さらに5HT_<1A>受容体に関しては9名の健常者の測定も行い、健常者との比較も行った。測定はm-ECTの前日、さらに1クール終了1日後、4週間後にPET検査、精神症状評価を行った。矩形波によるm-ECTの1クールは症状改善までで、週2〜3回の間隔で行った。5HT_<1A>受容体に関しては、(1)ECT施行前、(2)ECT施行後1日目、(3)ECT施行後4週間目の患者群と正常被検者の結合能を領域ごとに比較した。
D_2受容体に関しては治療の前後で結合能に有意な差は認められなかった。一方5HT_<1A>受容体については、m-ECT施行後1日目にほぼすべての皮質領域で20パーセント以上の結合能の増加が認められた。さらにECT施行前の患者群と健常者の比較では、前部帯状回、側頭葉内側部、島で患者群に有意な低値を認めた。しかしこの差はm-ECT施行後1日目には認められなくなった。これらの結果はm-ECTが5HT_<1A>受容体の機能変化を引き起こし、その変が抗うつ効果と関連している可能性があることを示唆するものと考えられた。
