Research Abstract

本年度は、当科で樹立している高悪性のヒト口腔扁平上皮癌株FIを用い、以下の項目について検討した。(1)in vitroにおいて、中性子線照射、ガンマ線照射、BNCTの生物学的効果を評価するためにコロニー形成法にて細胞生存曲線を求め、この曲線をもとにD_<10>、D_0を算出し、FI細胞における中性子の生物学的効果比(relative biological effectiveness : RBE)、およびBNCTの生物学的効果比(compound biological effectiveness : CBE)を算出した。(2)in vivoでは、下肢移植FI担癌ヌードマウスのに、通常投量より少ないBPA25mg/kgまたはBSHO.75mg/kgをip投与し、1時間、2時間、3時間後に中性子を照射し、腫瘍体積の変化、体重変化、白血球数の変化を経時的に観察し、本腫瘍に特徴的な腫瘍随伴症候群(cachexia, Leukocytosis)への影響を調べた。(3)更にこの研究を臨床に近づける為、in vivoにおいて頬部担癌ヌードマウスモデルを作成し、実際に中性子照射を顔面及び頸部に行ない、腫瘍増殖抑制効果、副作用、生存日数等を比較し、治療効果や副作用についても検討した。
その結果、それぞれ(1)口腔扁平上庫癌株FI細胞では、中性子のD_<10>におけるRBEは2.59、D_0におけるRBEは2.54であった。BNCTでは、BPAで最も強い殺細胞効果がみられ、BNCTのD_<10>におけるCBEは11.7、D_0におけるCBEは9.84であった。(2)BPA25mg/kgまたはBSH0.75mg/kg投与後照射群では、無処置群と比較して腫瘍が再増殖し腫瘍体積が両群間に差がないにもかかわらず、体重減少を抑制していた。また、腫瘍体積当りの血中の白血球数も、BPA投与群では無処置群に比較して、白血球増多症を起こしてはいるものの、白血球数に減少傾向が認められた。すなわち腫瘍随伴症候群の症状は、BPA-BNCT群では、改善していた。(3)頬部担癌ヌードマウスモデルにおいても、BSH-BNCT群は、非処置群と有意差がなかったが、BPA-BNCT群は腫瘍体積で約3週間、生存期間で約7週間延長できた。また、副作用としては、口内炎は特に出現しなかったが、長期生存したマウスでは、治療後6ヵ月頃より、白内障が認められた。以上の結果より、頭頸部癌においてもBNCTは有効であることが確認された。
