マイクロカプセルを用いたタクロリスム投与による新たな潰瘍性大腸炎治療法の開発

Research Abstract	潰瘍性大腸炎に対する免疫抑制剤の確実な効果と副作用予防のために、我々は、腸管免疫反応の重要な場である消化管リンパ装置を選択的に標的とする目的で、M細胞から取り込まれるマイクロカプセルを用いたDrug delivery system (DDS)を開発してきた。今回はこのマイクロカプセルに免疫抑制剤であるタクロリムスを含有させることに成功した。その後、潰瘍性大腸炎モデルとされる硫酸デキストラン(DSS)惹起腸炎やクローン病モデルとされる硫酸トリニトロベンゼン(TNBS)腸炎モデルに投与を行い、その治療効果を検討した。タクロリムス含有マイクロカプセル経口投与後の血中タクロリムスの濃度上昇は認められなかった。さらに、無治療群に比べて、治療群ではDSSおよびTNBSによる腸炎像は有意に軽減していた。この結果から、タクロリムス含有マイクロカプセルを用いた腸炎治療は全身におよぼす副作用は極めて少なく、局所における抗炎症効果はすぐれたものであると考えられた。さらに、無治療群に比べて、治療群の腸管組織中のMPO活性およびNO活性は有意に低下しており、TNF-α、IFN-γなどの炎症性サイトカインおよびこれらの炎症の発現に関わるとされている重要な転写因子の1つであるT-betおよびNF-kBの発現も低下していた。現在、よりヒト炎症性腸疾患に近いとされる自然腸炎発症モデルであるインターロイキン-10ノックアウトマウスに対してタクロリムス含有マイクロカプセルの経口投与を行い、その治療効果を検討している。
