免疫グロブリンの抑制性Fcレセプターを介した心筋炎・心不全治療に関する研究

Research Abstract	最近原因不明の自己免疫疾患や炎症性疾患での免疫グロブリン療法の有効性が注目されている。その作用機序は不明であるが、respiratory syncytialウイルス感染予防効果に代表される中和抗体としての作用、ITP(idiopathic thrombocytopenic purpura)での成功でみられるように、Fcレセプターのブロック作用、活性化された補体の不活性化や、炎症性サイトカインに対するdown-rregulatorとしての作用などが仮定されている。事実、これらの作用が複合的に働き、川崎病や多発性筋炎では著効している。さらに、川崎病に伴った心筋炎でも著効をみたとの報告が続いた。そこで申請者・岸本千晴は,その病状の進展に、病原としてのウイルスのみならず、炎症性サイトカインが関与している実験的心筋炎あるいは心筋症での詳細な検討を行い、その有効性を確認するとともに、免疫グロブリンのFc部分が効果発現に関与している結果を得つつある。そこで本研究では実験的心筋炎モデルならびに臨床症例で、免疫グロブリンの抑制性Fcレセプターを介する新しい治療の可能性を検討した。平成14年度は、動物実験を主体に検討した。免疫グロブリンのin vitroにおける効果の検討では、Fcレセプターのうちサイトカイン誘導などに関するnegativeなシグナルである抑制性Fcレセプターの関与が示された。次に、免疫グロブリンの動物モデルでの検討では、抑制性レセプターが免疫グロブリン療法のメカニズムであることが判明した。さらに、心筋障害での自己免疫機序に対する効果を検討すべく、現在ラットのミオシン感作型心筋炎モデルを用いて行っている
