神経芽腫に対する分子標的療法の基礎的研究と臨床展開

| Research Abstract |
本研究ほ神経芽腫に発現しているがん遺伝子産物HER-2/neuに対する分子療法の開発を目的としている。最近、成人のHER-2/neuを高発現している乳がんに対して、ヒトHER-2/neuに対するマウスのモノクローナル抗体をヒト型に改変(ヒトとマウスのキメラ抗体)した製剤(trastuzumab)が開発され、2001年6月に保険収載された。HER-2/neuは、1980年代前半にラットの神経芽腫において発見されたがん遺伝子である。ヒト神経芽腫におけるHER-2/neuの発現を調べた報告は少なく、また神経芽腫細胞に対するtrastuzumabの効果を調べた研究はない。今回我々は、神経芽腫の臨床検体を用いてHER-2/neuの発現を調べた。
ステージ4の神経芽腫患者より、両親から同意を得た後、骨髄細胞を採取する。比重遠心法により単核球を得た後、Engelbreth-Holm-Swarm Natrixを塗布したフラスコに入れ、培養を開始する。付着細胞の細胞表面のGD2をフローサイトメトリーにより調べ神経芽腫患者から得られた腫瘍細胞に対して免疫染色を行いHER-2/neuの発現を調べたところ、8例中1例において中等度の発現を認めた。この症例において、神経芽腫細胞を短期培養し、trastuzumabに対する感受性をADCC(antigen-dependent cell-mediated cytotoxicity)アッセイにより検討したが、顕著な効果は認められなかった。今後さらに症例数を増やして研究を進める予定である。
