Research Abstract	成人発症スティル病の登録用紙を作成、各研究分担者へ郵送した。当科、長崎原爆病院、佐賀医科大学内科、北海道大学大学院医学研究科分子病態制御学講座(第二内科)、埼玉医科大学総合医療センター第二内科、東北大学医学部血液免疫科、東京医科歯科大学医学部膠原病・リウマチ内科、鹿児島大学医学部小児科、の施設から現在までに計64例の回答があった。【結果】アンケート結果をこれまでの日本での報告(90例)、海外での報告(228例)(Ohta A et al.:J Rheumatol 14:1139-1146,1987・Ohta A et al.:J Rheumatol 17:1058-1063,1990のデータを参考にした)と比較した。検査所見については白血球増加(1×10^4万以上)は75.0%(89%、91.7%)、抗核抗体陰性70.0%(93%、92.8%)とやや低かった。フェリチン値の平均は9453.5ng/ml、sIL-2R値の平均は3877.3U/mlであった。臨床経過では単周期性が35.9%、慢性関節炎を伴った単周期性が3.1%であった。臨床特徴については、発熱100%、関節痛(関節炎)89.1%で他の報告と大差なかったが、他の特徴として定型的皮疹65.6%(日本90例中87%、海外228例中87.5%)、リンパ節腫大48.4%(69%、60.3%)、肝腫23.4%(48%、40.6%)、胸膜炎4.7%(12%、24.6%)、心外膜炎4.7%(10%、12.8%)、腎障害1.6%(12%、10.7%)、薬物アレルギー17.2%(54%、記述なし)と低い結果であった。初期治療についてはプレドニゾロン内服が61例と最も多く、著効24例、効果あり35例と初期治療効果ありがほとんどを占めた。死亡例は自殺の1例のみであった。転帰も治癒8例、軽快52例と良好であった。以上より、アンケートに含まれる症例の妥当性としては適当であると考えられた。これらはデータベースに入力した。今後、高サイトカイン血症と本症の病因・病態との研究へと展開予定である。
