成人チアノーゼ性心疾患児の血管内皮機能ビタミンC投与による新しい治療的試み

Research Abstract

<背景>先天性心疾患に対する心臓外科手術の進歩した現在においても、心疾患に対する根治術の適応が無くチアノーゼが残存した状態で日常生活を過ごして症例は本院においても少なからず存在している。チアノーゼが残存した症例では思春期からヘマトクリット上昇に伴う過粘稠度症候群による頭痛、腹痛などの不定愁訴を訴え、さらに成人期に達すると合併症として高尿酸血症、脳梗塞、消化管・気道からの出血などが出現しする。これらの症状は日常生活を著しく妨げており、チアノーゼ性心疾患患者の長期予後は決して芳しいものでなく加齢とともに症状が悪化しゆき、対処療法のみで経過観察しているのが現状である。
<目的>青年期チアノーゼ性心疾患児における血管内皮機能の評価を行い、ビタミン(Vit.C)が血管内皮機能に与える効果を検討した。対象は12例のチアノーゼ性心疾患児(17.0土1.4歳、男:女6:6)と年齢と性別を一致させた対照群12例とした。血管内皮機能は内皮依存性の血管拡張能であるFlow-mediated dilatation (FMD)により評価した。
<結果>1)2群間のFMDの比較はチアノーゼ群の有意な低下を示した(3.3±4% vs.9.1±6%、p<(0.05)。2)チアノーゼ群と対照群におけるニトログリセリン(0.6mg)舌下投与により両群共に良好な血管拡張を示した。3)Vit.C投与によりFMDは両群共に改善した(12±5% vs.15±9%,p(0.05)。
<結語>青年期チアノーゼ性心疾患児における血管内皮機能低下が認められ、抗酸化作用を持つVit.C投与は血管内皮機能を改善することが示唆された。
