薬物代謝と生理活性脂質の生成に関与するシトクロームＰ－４５０の薬理遺伝学的解析

Outline of Annual Research Achievements

クロピドグレルは、ステント挿入による経皮的冠動脈形成術（PIC）において、動脈硬化を抑制するために一般的に広く使用されている抗血小板性P2Y12受容体阻害薬である。しかし、クロピドグレル自体は不活性な前駆薬剤であり、薬効を示すためには生体内に吸収されて、シトクロームP-450のイソ酵素により酸化、加水分解される代謝変換を受けなければならない。今回の研究では、バングラデシュにおいて、PICの手術を受けている患者の血液を用いて、ゲノムDNAを抽出し、複数のシトクロームP-450のイソ酵素類（CYP）、また、薬剤の効能に関連するパラオキソナーゼ-1 (PON-1)、さらに、P-糖タンパク質であるABC輸送体に関して、標的の遺伝子をポリメラーゼ連鎖反応で増幅し、特異的な制限酵素切断による制限断片長多型や塩基配列の決定による遺伝子の一塩基多型の解析を実施している。これまでに、CYP3A4*1B、ABCB1*C1236T、PON-1の遺伝子型や対立遺伝子の解析結果を得た。それによると、解析したPIC患者の試料では、CYP3A4*1BのAG遺伝子型が4.6%であり、GG遺伝子型は認められなかった。同様に、ABCB1*C1236Tでは、CCとTCの遺伝子型が、それぞれ46.4%と40.9%となった。また、PON-1のAGとGGの遺伝子型は、それぞれ44.6%と24.6%であった。さらに、現在、クロピドグレルの代謝と作用に影響する10種類以上の関連遺伝子の多型解析を行っている。また、患者血液中の薬物の代謝産物を分析する検討も行っている。さらに、関連研究として、乳癌の発生に関係する遺伝子多型、薬用植物の生理活性物質の解析、脂肪細胞でのアラキドン酸代謝反応に関する研究にも関与した。

Reason

現在にまでに、目的の標的遺伝子の遺伝子多型解析を順調に進行しており、薬物の代謝産物の解析のための分析機器を用いた研究も行っている。また、解析した遺伝型のデータを蓄積して、表現型の解析データに関連できると考えている。

Strategy for Future Research Activity

今後、シトクロームP-450イソ酵素、ABC輸送体、さらにP2Y12受容体などについて、さらに、複数の標的遺伝子の一塩基多型解析を行う。また、患者血液中のクロピドグレルの代謝産物の濃度を超高速液体クロマトグラフィー質量分析計により定量分析する予定である。これらに研究により、標的遺伝子の一塩基多型と薬物代謝の表現型との関連性を明らかにする。また、脂肪細胞の機能を制御する生理活性脂質の関連研究も継続する。
