Outline of Annual Research Achievements

本年度の研究では、高齢者における運動イメージ能力低下と2重課題歩行時の歩行機能低下の関連の背後にある神経活動変化について明らかにすることを目的とした。
41名（平均年齢74歳、女性19名）の地域在住高齢者が実験参加に同意し、歩行機能測定、運動イメージ能力測定、脳容量解析のためのMRI検査を受けた。歩行機能の測定には、6mのマット式の歩容解析機器を用いた。歩行時には、通常歩行に加えて、100から1ずつ減算する課題と100から7ずつ減算する課題を2重課題として行った。解析には歩行速度、歩幅、1歩行周期当たりの時間に加えて、歩幅と1歩行周期時間の変動係数を用いた。運動イメージの測定には、Timed Up & Go test（イスから立ち上がり3m先の指標を回り、再びイスに座るまでの時間を測定、以後TUGテスト）を用いた運動時間予測課題を用いた。MRI撮像には3テスラMRIを用い、VBM（Voxel Based Morphometry）を用いて脳容量とそれぞれの変数との関連性について解析を行った。実験の結果、これまでの研究結果同様、運動イメージ能力と2重課題時の歩行能力の間に有意な関連が認められた。これらの成績と脳容量の関連を調べたところ、運動イメージ能力と有意に相関する脳部位は認められなかった。他方、2重課題時の歩行能力と海馬容量との有意な相関関係が認められ、2重課題時に歩行速度が低下する、もしくは歩幅・1歩行周期時間の変動係数が高くなる者ほど海馬の容量が小さいことが明らかとなった。しかしながら、運動イメージ能力を共変量として投入するとこれらの有意な相関関係は消失した。以上の結果は、2重課題時の歩行能力低下は海馬萎縮の前駆症状であり、その関連には運動イメージ能力が強く関与している可能性を示唆している。
