| Research Abstract |
(1)本研究計画について、倫理委員会での承認後、被験者の募集を行った。
(2)被験者のうち、最近6か月間に3回/週以上の歯ぎしり、起床時における咀嚼筋の疲労感や違和感ならびに歯の咬耗が認められる者をブラキシズム患者とし、これらの症状ならびに顎関節症状を共に認めない者を正常者とした。
(3)睡眠時の両側咬筋の筋活動の記録と顎顔面運動ならびに睡眠時の体位の記録を被験者の自宅で行い、4回/時以上のブラキシズムエピソード、25回/時以上のブラキシズムバーストならびに2回/晩以上の歯ぎしり音のある者をブラキシズム患者とし、それ以外を正常者とする最終診断を行った。
(4)ポリソムノグラフ、食道内pH・運動モニタリングシステム、8チャンネル生体アンプ・メディカルトライシステムならびにビデオカメラを用いて、食道内pH、食道下部括約筋圧、脳波、眼電図、心電図、呼吸、筋電図(側頭筋)、体位、音声・画像の記録を同時に行った。pHセンサーならびに内圧センサーは、内圧データによってその位置を確認しながら、下部食道括約筋相当部に設置した。これらの記録を被験者の自宅で2晩連続して行い、2日目のデータを解析データに用いた。被験者には、検査期間中、決まった食事を同じ時間帯に摂ってもらった。
(5)ブラキシズム・上部消化管機能解析ソフトウェアと音声・画像データを用いて、ブラキシズムエピソード、酸逆流エピソードならびに下部食道括約筋弛緩エピソードを判定した。
(6)脳波と筋電図データなどを用いてトレーニングを受けた検者が睡眠段階を判定した。
以上のエピソード間の時間関係を統計学的に比較検討して、睡眠時において、食道下部括約筋の弛緩すなわち胃食道酸逆流によって引き起こされる食道内pHの低下時にブラキシズムが特異的に生じることが示唆された。
