| Research Abstract |
平成15年度では,病理組織学的に口腔扁平苔癬と確定診断を得た患者におけるHBs抗原(B型肝炎ウィルス),HCV抗体(C型肝炎ウイルス)の陽性率を血清および唾液において検討した.対象として口腔癌患者,口腔内嚢胞患者も同時に測定した.口腔扁平苔癬患者ではHBs抗原(B型肝炎ウィルス),HCV抗体(C型肝炎ウイルス)ともに,口腔癌患者,口腔内嚢胞患者よりも陽性率が高い傾向が認められた.なお,それぞれの疾患の血清GOT, GPT,γ-GTPの肝機能の差異は認められなかった.以上のように,口腔扁平苔癬患者では肝炎ウィルスによる感染が影響を与えていることが示唆された.一方,唾液中のHBV抗体とHCV抗体をそれぞれHBV-DNA, HCV-DNAのPCR産物で定量した.その結果,血清中HCV抗体陽性者と唾液中HCV抗体陽性者の一致率は95%以上であったが,唾液中にはC型肝炎ウィルスは検出できなかった.これに対しB型肝炎ウィルスは唾液中にも存在することが判明した.したがって,B型肝炎ウィルスの方がC型肝炎ウィルスよりも口腔粘膜や口腔扁平苔癬に影響を与えているものと推測された.さらに,これまでに歯科疾患との関連を明らかにした新しい肝炎ウィルス(G型,TTV型)感染と口腔扁平苔癬との関連を検討中である.
