自己免疫性膵炎(IgG4関連膵炎)感受性遺伝子の検索

| Research Abstract |
1994年から2002年までの間に診断した自己免疫性膵炎40例を対象とし、TNF-α遺伝子promoter多型、IκBL1遺伝子promoter多型についてそれぞれdirect sequencing,PCR-SSCPによるスニップ解析を行った。HLA領域における疾患感受性遺伝子の検索はHLA領域内に設定した19種類のマイクロサテライトマーカで相関解析を行った。
慢性関節リューマチにおけるsecond susceptibility geneであるIκBL1遺伝子はHLA class III領域に存在する。promoter領域の5種類の多型との関連を検討したが、自己免疫性膵炎と有意な相関を認めなかった。また種々の自己免疫性疾患との関連が報告されているTNFαプロモーター領域多型との関連を検討したが、有意な相関を認めなかった。しかし、これらclass II抗原をもたない患者も存在することより、HLA領域におけるHLA遺伝子以外のsecond susceptible geneを検索する目的で、マイクロサテライトマーカーを用いてHLA領域の疾患感受性遺伝子マッピングを検討したが、DRB1*0405とDQB1*0401領域以外には、顕著な感受性を示すcandidate geneの存在は認められなかった。従って、自己免疫性膵炎ではHLA DRB1*0405-DQB1*0401 haplotypeがHLA領域における疾患感受性遺伝子と考えられる。今後はHLA領域以外について、マイクロサテライトやSNPを用いたゲノムワイドな解析により疾患感受性遺伝子の検索を行っていく予定である。
