| Research Abstract |
滋賀医科大学付属病院に急性心筋梗塞にて入院した患者を対象とし、冠動脈造影検査および冠動脈形成術を行った。3ヶ月後に、再狭窄精査のために心臓カテーテル検査を施行した。冠動脈内皮機能検査としてアセチルコリン負荷試験およびブラジキニン負荷試験を行った。ドプラーガイドワイヤーを左冠動脈前下行枝の近位部に留置し、薬物投与前に冠血流速度、採血、冠動脈造影を施行し、左前下行枝内にブラジキニン(0.2,0.6,2.0μg/min)を各2分間、アセチルコリン(3,10,30μg/min)を各2分間投与した。それぞれ投与終了後に冠動脈造影を施行した。定量的冠動脈造影と左前下行枝の冠血流量の測定により冠血流量を求めた。心筋梗塞発症後2週間に心筋シンチグラムを施行した。テクネシウム製剤を静脈注射し、心電図同期下にSPECTを撮像した。発症後3ヶ月後に運動負荷心筋シンチグラムを施行した。運動負荷はトレッドミルを用いて行った。運動直後および、3時間後にSPECT撮影を行った。心電図同期シンチグラムにより心機能および虚血の評価を行った。得られたSPECT像は短軸の8分割し、心尖部よりと心基部よりの16領域と心尖部の2領域を加えた16領域を視覚的に4段階にスコア化した。Germanoらが開発したQuantitative Gated SPECT(QGS)を用いて心機能の解析を行った。研究者は研究課題を着実に遂行している。また、血管内皮関連、心筋シンチグラム、心筋代謝などの研究について英文誌において発表を行った。心筋梗塞患者のQGSを用いた予後評価について第43回日本核医学会総会において発表を行った。
